Celiac disease reveals an autoimmune reaction to gluten — specifically gliadin peptides — creating damage to the small intestine! This involves HLA-DQ2/DQ8 genetics, tissue transglutaminase antibodies, and villous atrophy disrupting nutrient absorption!
HLA-DQ2 (present in 90-95% of patients) or HLA-DQ8 (5-10%) are necessary but not sufficient! These MHC class II molecules present gliadin peptides to T cells. Antigen-presenting cells present modified gliadin peptides via HLA-DQ2/8 to CD4+ T cells in the lamina propria! These T cells produce IFN-gamma, TNF-alpha, and IL-15, activating inflammation. Intraepithelial lymphocytes (especially gamma-delta T cells) proliferate, expressing the NKG2D receptor that recognizes stress markers on epithelial cells!
B cells produce antibodies against tissue transglutaminase (anti-tTG), endomysium (EMA), and deamidated gliadin! tTG-gliadin complexes disrupt tolerance. Anti-tTG antibodies can directly damage intestinal architecture. IgA antibodies predominate (testing must screen for IgA deficiency, which is common in celiac disease!)
Villous flattening and crypt hyperplasia dramatically reduce absorptive surface area! Enterocyte damage creates malabsorption of iron, calcium, folate, and fat-soluble vitamins. Increased intestinal permeability allows entry of bacterial antigens. Dermatitis herpetiformis — an itchy skin manifestation — reflects IgA deposition in the dermis!
