Daytime sleepiness (excessive somnolence) occurs when the brain's arousal systems fail to maintain adequate wakefulness during appropriate hours. This can result from insufficient sleep duration, poor sleep quality, circadian rhythm disorders, sleep disorders (apnea, narcolepsy), medications, or medical conditions. The locus coeruleus (which releases norepinephrine for alertness) and histaminergic neurons (which promote wakefulness) show reduced activity. Adenosine (a sleep-promoting chemical) accumulates excessively in the brain when sleep debt persists, creating overwhelming pressure to sleep. The orexin/hypocretin system (which stabilizes wakefulness) may be impaired.
