OCD reveals hyperactivity in the orbitofrontal cortex (OFC), anterior cingulate cortex (ACC), and caudate nucleus creating persistent obsessions and compulsions! This involves serotonergic dysfunction, error signal amplification, and habit loop entrenchment!
The orbitofrontal cortex generates error signals — "something is wrong"! The caudate nucleus normally filters these signals, allowing only relevant ones through to the thalamus. Glutamate levels in the caudate and anterior cingulate are elevated! NMDA receptor hypofunction may contribute. N-acetylcysteine (a glutamate modulator) shows benefit as augmentation. Memantine (NMDA antagonist) may help treatment-resistant cases!
Exposure and response prevention (ERP) retrains the circuit through habituation! SSRIs (fluoxetine, fluvoxamine at higher doses) modulate serotonergic function! Deep brain stimulation of the ventral capsule/striatum disrupts the hyperactive loop! Cognitive therapy challenges catastrophic appraisals!
